Medical & Health and Liability Disclaimer
In only limited classes we shall provide a qualified nurse to assist during the workshop. The nurse will be available to offer basic medical assistance and support. While she will try to provide limited assistance, we cannot guarantee the prevention of injuries, medical issues incidents or accidents. Participants and their parents understand that they attend the workshop at their own risk.
Medical Information
I hereby certify that my child ________ is physically able to participate at Young Leaders. I authorize Young Leaders and its representatives to obtain medical treatment for my child in case of an emergency. I understand that I am responsible for any and all medical expenses that may be incurred on behalf of my child. I agree to release, discharge, and hold harmless Young Leaders (Young Leaders Education Services Private Limited), its officers, directors, employees, agents, and representatives from any and all claims, demands, causes of action, or liability arising out of my child’s participation in the skill-building programs.
Transportation service
The bus service does not provide pick-up and drop-off directly at children’s homes or specific chosen locations. Instead, it follows a fixed route with scheduled stops, ensuring children are picked up and dropped off at designated points on time. However, this may change due to some unavoidable factors.
I give my permission for my child to be transported to and from all scheduled activities planned and organized by Young Leaders at different locations in Delhi-NCR by its officers, employees, agents, and representatives, or by a private carrier hired by the organization under proper security systems. Young Leaders will provide a fixed-point pick and drop facility for the convenience of parents and guardians. I understand that the pick and drop facility will adhere to strict security protocols to ensure my child’s safety.
Changes to this Privacy Policy
We may update this Privacy Policy from time to time to reflect changes in our practices or for legal reasons. We will post the updated Privacy Policy on our website, and the revised date will be indicated at the top.
Consent form
I, _____________________________, hereby give my consent for my child, _______________________, to participate in the 104 Skill Building Program at Young Leaders. I understand that participation in this program involves certain risks and hazards, including but not limited to physical injury or harm, and I accept these risks on behalf of my child. Young Leaders will provide the security and safety measures while conducting all indoor and outdoor classes.
